Example: damaged-on-arrival complaints after a supplier change
Illustrative example, not customer data. This is page 1 of the template; page 2 is the same plan, blank.
| Section | Example | Evidence |
|---|---|---|
| Problem | Damaged-on-arrival complaints rose from 1.2% to 4.8% of orders in the three weeks after the switch to a new packaging supplier | Ticket tags, order data |
| Impact | 486 complaints on 10,125 orders in week 3; 394 of them (81%) on orders over 2 kg. Replacements and reshipping cost about $15,100 that week. | Ticket tags, finance |
| Containment | Within 48 hours: all 6,400 new-supplier boxes in stock inspected and quarantined (single-wall, not double-wall); orders over 2 kg packed in the old boxes or double-boxed; 1 in 20 heavy parcels checked before pickup; customers with open complaints get the replacement first | Warehouse inspection log |
| Root cause | The spec sent to the new supplier listed box sizes but not board grade, so the boxes came single-wall. The supplier change process had no transit test before new packaging went live. | Old boxes 1.1% damaged (20 of 1,800 orders), new boxes 5.6% (466 of 8,325); drop test crushed 7 of 10 new boxes |
| Corrective action 1 | Replace all single-wall boxes with double-wall boxes from the new supplier. Owner: procurement manager, due 5 Jun. | |
| Corrective action 2 | Add board grade, crush rating and maximum load to the spec for every box size. Owner: packaging engineer, due 3 Jun. | |
| Preventive action 1 | Transit test (drop and compression, three heaviest products per box size) before any new packaging goes live. Owner: QA manager, due 19 Jun. | |
| Preventive action 2 | Damaged-on-arrival rate reported every week by box size, with an alert above 2%. Owner: operations analyst, due 12 Jun. | |
| Success metric | Damaged-on-arrival complaints at or under 1.5% of orders at every check (baseline 1.2%, peak 4.8%) | |
| Closure | PASS at 30, 60 and 90 days. Closed on 7 Sep by the operations manager and the QA manager. | Effectiveness check |
Effectiveness check: 30, 60 and 90 days after the new boxes were in use (8 Jun)
| Check | Date | Damaged-on-arrival rate | Target | Result |
|---|---|---|---|---|
| Day 30 | 8 Jul | 1.4% | ≤ 1.5% | PASS |
| Day 60 | 7 Aug | 1.3% | ≤ 1.5% | PASS |
| Day 90 | 6 Sep | 1.2% | ≤ 1.5% | PASS |
The 5 steps of corrective action
- 1Describe the problem and its impact: what, where, since when and how much, against a baseline. Keep causes out of it.
- 2Contain it within 24 to 48 hours: inspect or quarantine suspect stock, add a temporary check, and look after the customers already affected.
- 3Find the root cause and verify it with data: it should explain when the problem started, and where it did not happen.
- 4Plan corrective actions that remove the cause and preventive actions that stop it happening elsewhere, each with an owner and a due date.
- 5Verify effectiveness and close: set the metric and target first, check at 30, 60 and 90 days, and close only if every check passes.
Corrective vs preventive action (CAPA)
CAPA stands for corrective and preventive action. Containment protects customers today; the corrective action removes the cause of this problem; the preventive action stops the same kind of problem happening again or somewhere else.
| Corrective action | Preventive action | |
|---|---|---|
| Question it answers | How do we stop this problem coming back? | Where else could this happen, and how do we stop it there? |
| Acts on | The verified root cause of this problem | The process or system that let the cause in |
| In the example | Double-wall boxes again; board grade written into the spec | Transit test before any new packaging goes live; weekly damage rate by box size |

Frequently asked questions
Is the corrective action plan template free?
Yes. The printable PDF downloads without an email. The Excel pack and the Word CAPA report are sent to your email for free.
What is a corrective action plan?
A record of how a specific problem gets fixed for good: the problem and its impact, the containment that protects customers, the verified root cause, corrective and preventive actions with owners and due dates, and the check that proves the fix worked.
Is this an employee corrective action plan?
No. This template is for process and product problems, such as a rise in customer complaints, defects or service failures. It is not an employee-discipline or performance-improvement plan and should not be used as one.
Is a CAPA report the same as a corrective action report or form?
Mostly, yes. A corrective action report or form records the problem, the root cause and the fix. A CAPA report adds preventive actions and the effectiveness check. This template covers all of it, so you can use it for any of the three.
How do you know a corrective action worked?
Choose one metric, its baseline and a target before you act. Check it 30, 60 and 90 days after the fix is in place, and close the plan only if every check meets the target. If one fails, go back to the root cause before you add more actions.
Is there a Word, Excel or Google Sheets version?
Yes. The pack has an Excel workbook with five sheets and a Word version of the CAPA report. For Google Sheets, upload the .xlsx file to Google Drive and open it with Google Sheets; the dropdowns, PASS / FAIL formulas and overdue highlighting carry over.
What if the complaints come in by the thousands?
Step 3 is where most plans go wrong: a workshop works from a sample and from memory. Pivony runs root cause analysis on all of your customer feedback, so the plan starts from the full picture.
Have thousands of complaints, reviews or tickets?
